DPC Monthly Membership FAQs

What is Elevate Medical?

  • Elevate Medical is Livingston’s first Direct Care practice. We offer a pattern of family health care that is immediate, thorough, comprehensive, specific, easily affordable, and very personal.

What is DPC?

  • Direct Primary Care or DPC is a new model of primary care that puts the patient first and cuts out the insurance middleman avoiding the red tape that restricts traditional primary care practices.

Is concierge medicine the same as direct care?

  • The terms are often used interchangeably and essentially refer to the same principles. Direct Primary Care is a healthcare model focused on putting the patient first. That means treating patients when they need it, never rushing them through appointments, and being proactive with health care treatment plans. The doctor-patient relationship is just as the name suggests – direct. Direct Primary Care clinics don’t accept insurance, rather they choose to work directly with the patient, providing wholesale labs, deeply discounted prescription prices, and other cost savings along the way.

How is Elevate Medical different than other practices?

  • At Elevate Medical we spend all of the time you need to give you the care you deserve. We have taken primary care back to its roots putting the physician-patient relationship at the front. You have access to your doctor when you need them, you don't have to wait to be seen, you can get an appointment same or next day. You have access to phone, text, email, telehealth, video visit, or an in-person visit to speak with your doctor.

Will I benefit from Elevate Medical if I don’t require frequent medical attention?

  • Of course! We offer accessible and personalized health care that everyone can benefit from. We help you develop a personalized health and wellness plan. We are devoted to the maintenance of your good health to enhance your daily experience in a way that only physical well-being can bring. We are here for your acute care needs and preventative health care needs. Even if you don't require frequent medical attention now, your health needs may change in the future. By establishing a relationship with Elevate Medical, you have a consistent source of care that knows your medical history, preferences, and specific health care needs.

Does Elevate Medical accept health insurance?

  • We accept patients with all types of insurance and we can use your insurance for the “big stuff” like imaging, labs, medications and referrals. What we don’t do is bill your insurance for office visits, strep or flu tests or in office procedures. For your care here in our office we leave out the middleman who has no real vested interest in you as a person or a patient and we remain faithful to the doctor-patient relationship. Perhaps the greatest distinction between Elevate Medical and other primary care offices is the relationship between you and your doctor. This allows us the freedom to provide the care you need without the say of a third party. It also allows us the ability to provide wholesale prices on medication, labs, and imagining.

As a patient at Elevate Medical do I still need health insurance?

  • Yes. We recommend our patients continue a major medical plan with a high deductible and a health savings account to ensure financial help should hospitalization or referral to a specialist be necessary. We’re happy to refer you to insurance representatives who can help customize an insurance plan to your specific needs.

If I have Medicare can I still be a patient at Elevate Medical?

  • Yes. You need only sign a one-time waiver declaring that neither you nor your doctor will directly bill Medicare for our services. Medicare will still cover any laboratory testing, imaging, medications, or hospitalizations prescribed by our offices.

What if I need care while I am away from home?

  • Naturally we provide such care. This is yet another freedom we enjoy together as a result of our independence from the restrictions of third-party payment. Because we know you well, we can streamline your care when you’re sick and away from home. Many illnesses can be diagnosed and treated with a simple conversation by phone, email, text, or webcam. We’ll locate the nearest pharmacy and order the medications most appropriate for your circumstances.

How will I know if the doctors of Elevate Medical are right for me?

  • Come get to know us! Schedule a free, unconditional visit to discuss your health care needs. It’s the only reasonable way to begin what we trust will be a long, productive, healthy relationship. Get in touch here!

About Elevate Medical

The word elevate means to lift to a higher level. At Elevate Medical we raise the bar and the standards for your health care to the highest level. With this in mind, it is an opportunity to provide you with the highest quality direct and personalized health care for a set monthly membership fee. By eliminating insurance companies from the equation, you won’t have to worry about confusing coverage limits, claims, co-payments, or deductibles – and in turn it allows Dr. Cahill to practice outside of the insurer imposed box. This form of concierge medicine and personalized attention from a board-certified physician, helps lead to greater satisfaction for both members and physicians. It also leads to improved health and outcomes. With more face-to-face time, Dr. Cahill’s goal is to focus on managing health through lifestyle and personalized care.

Why I started Elevate Medical

I decided to start a DPC (Direct Primary Care) practice because I wanted to provide a higher level of personalized and comprehensive care for my patients. I was attracted to the DPC model because it allows me to spend more time with each patient, which ultimately results in better health outcomes.

Operating outside of the traditional insurance model means that we avoid the administrative burdens that often come with insurance billing and cut out the middleman. By starting a DPC practice, I hoped to create a more fulfilling and rewarding experience for both myself and my patients, resulting in improved overall well-being!

Cough, Colds & Congestion

This information is intended only for patients with an established relation ship with Dr. Cahill. Patient should always use their best judgement to seek medical attention directly with a physician for worrisome ongoing symptoms. This Patient is not intended to replace direct consultation or care.

Congestion, ear pressure, cough, sore throat, Laryngitis, Bronchitis

The first 7-10+ days of cold, sinus, bronchitis and laryngitis symptoms are nearly always viral. Symptoms usually peak around 5 days. Thicker yellow/green mucus is common. Antibiotics do not improve viral infections, and may cause troublesome side effects such as diarrhea and bacterial resistance that can lead to later dangerous bacterial infections that will not respond to antibiotics.

A mild cough can linger after a viral infection for several weeks, as residual inflammation heals. If beyond the 10 day mark, you note a cough that is worsening, new fever >100.4, new wheezing, shortness of breath or focal chest or facial pain: please contact your doctor. However, in the absence of these, it’s generally still a “supportive cares” plan as below.

Infant & Child Symptom Care

1. STEAMY SHOWERS to moisturize and thin mucus.

2. NASAL SALINE: Mist or 1-2 drops 2-3 times daily until symptoms improve to moisturize passages and keep mucus thin. May use bulb suction a minute or so after saline use to pull mucus out and clear the nose.

3. NASAL SPRAYS: kids that are preschool aged and older may tolerate OTC fluticasone (Flonase) or other OTC nasal steroid: 1 spray each nostril daily; May continue the steroid spray as long as needed. These help keep nasal and sinus passages open so that mucus can drain (preventing bacterial sinus infection), and inner ear pressure/pain is relieved. It can be drying: ok to use a little vaseline just inside the nose if noting nose bleeds.

4. ACETAMINOPHEN (Tylenol) every 4-6 hours and/or IBUPROFEN (Motrin) every 6-8 hours as needed for sore throat, ear pain, fever or body aches. You can give whichever allows your child the HIGHER dose: 1) age based – or – 2) weight based

IBUPROFEN (Motrin) DOSAGE BY WEIGHT/AGE

ACETAMINOPHEN (Tylenol) DOSAGE BY WEIGHT/AGE

5. HONEY (**ONLY for kids >12 months) is also proven to help lessen cough and soothe the throat. A spoonful 2-3x/day.

OLDER TEEN & ADULT SYMPTOM CARE

1. STEAMY SHOWERS to moisturize and thin mucus.

2. NASAL SPRAYS: Afrin nasal spray: 2-3 sprays/nostril, twice daily for up to 4 days for nasal congestion. OTC fluticasone (Flonase) or other OTC nasal steroid spray about 5min after the Afrin: 1-2 sprays each nostril daily; May continue the steroid spray as long as needed. These help keep nasal and sinus passages open so that mucus can drain (preventing bacterial sinus infection), and inner ear pressure/pain is relieved. Vaseline via qtip just inside the nose to swab the septum to can lessen bloody noses from any over-drying if this occurs.

3. NASAL SALINE: Spray, drops, Neti-pot or SinuCleanse twice daily until symptoms improve to moisturize passages and keep mucus thin.

If using Neti-pot or SinuCleanse, use lukewarm water, and 1/2 salt packet at first to avoid burning sensation in nose. Do *not use standard table salt.

4. SUDAFED (pseudoephedrine) by mouth, as directed on box for nasal congestion. If you are treated for high blood pressure, you may need to avoid over the counter oral decongestants which can raise blood pressure. You’ll need to ask the pharmacist for this, but it is still OTC.

5. ACETAMINOPHEN (Tylenol) 1000mg 4x/day or IBUPROFEN 400-600mg 4x/day for sore throat, facial pain, fever or body aches for older teens and adults. For kids 12yo and under: dose can be the greater of the age or weight based as per OTC box instructions.

6. CEPACOL lozenges or throat spray and salt water gargles for sore throat.

7. MUCINEX DM (guaifenesin/dextromethorphan) as needed for cough

8. HONEY is also proven to help lessen cough and soothe the throat. A spoonful 2-3x/day.
